There are some essential considerations that should guide your evaluation of how well COLLAGE fits your organization:

  • What types of client assessments are currently used? Are all clients assessed in “independence?” If not, what is the criteria that triggers some type of assessment?
  • Do you have a tool that integrates and standardizes your clients’ information across service lines or sectors?
  • Are you confident that a high percentage of your independent residents’ health and wellness is tracked and managed consistently?
  • Where did your assessment tools come from and how much rigor was applied in their development?
  • How does your organization use health and wellness outcomes to inform program/service development, and advance and promote healthy aging?
  • Do you have systematic ways to make informed data-based choices about client risk and level of care/service decisions ?
  • Do you have a way to benchmark your health and wellness indices against other similar aging services organizations? Is this important to you?
  • Do you have a reliable way to facilitate improved individualized service plan development?
  • Do you have a way to evaluate health and wellness outcomes for your independent living, home care or housing clients? Does it matter to you?
  • How does your organization plan for and target specific campus-wide or system-wide needs and interests of your clients?
  • How are you ramping up programs and services earlier in your clients’ lives to facilitate adaptation and aging in place more gracefully?

  • The width of the content display panel is FIXED at 504 pixels wide (inside the display margins). Graphics and other inserts (e.g. movies, etc.) can not exceed this width.
  • Where possible, use the Paragraph tag to wrap text and image inserts in order to keep proper spacing and alignments.
  • On FORM pages you can only edit/change the title graphic, field captions and the text above the form.

Keep in mind that you must update the Site Navigation File if:

  • You have created a new web page from one of the pre-defined page templates
  • You want to change where the page is displayed in the site's navigation menu
  • You want to restrict the page viewing to Members ONLY
  • There is no need for the fixed-height scroller display (all of your content for this page is above the bottom of the page's picture graphic (not the reflection)

Please note that when this page is delivered to a browser on the web, the top and bottom menus will be dynamically valued, the content panel to the right will be a fixed height and this information panel will NOT be displayed.
