The Health and Social Check-up includes four broad focus areas including: Community and Social Life, Cognition/Mental Health, Everyday Life, and Health. The Health and Social Check-up gathers key information about the individual and when used with the Lifestyle Survey provides a holistic perspective. Data coming from this assessment tool informs organizations about their resident’s health and wellness needs and potential risks to independence, and allows them to better target programs and resources to enhance healthy aging. Information from the Health and Social Check-up, Lifestyle Survey, and Vitality Plan is entered into a web-based software system and used for organizational outcome measurement, benchmarking and quality improvement.

The program embraces the entire well-being of a person — the 360 degree view — and encourages individuals to consider all aspects of their lives including sense of purpose, lifelong learning, social connections, community activity, mental and physical fitness, nutrition and health. The coaching conversation is designed to support existing activities, inspire new achievements, and foster a meaningful, active lifestyle towards living the best life possible.

Keep in mind that you must update the Site Navigation File if:

  • You have created a new web page from one of the pre-defined page templates
  • You want to change where the page is displayed in the site's navigation menu
  • You want to restrict the page viewing to Members ONLY
  • There is no need for the fixed-height scroller display (all of your content for this page is above the bottom of the page's picture graphic (not the reflection)

Please note that when this page is delivered to a browser on the web, the top and bottom menus will be dynamically valued, the content panel to the right will be a fixed height and this information panel will NOT be displayed.
